In the finale of St. Elsewhere, it was revealed that the entire show took place inside the imagination of an autistic boy named Tommy Westphall. Because hundreds of shows crossed over with St. Elsewhere — and crossed over with each other — they are all arguably figments of Tommy's imagination too.
